2020 Entry Forms

 

In an effort to simplify the release form collection process on our end, we have created different Entry Forms for each submission category. The Entry Form itself hasn’t changed but you will need to complete the correct Entry Form based on the submission category you are submitting in (Suicide Prevention, Mental Health Matters, Animated Short, SanaMente, Through the Lens of Culture, or Walk in Our Shoes). Remember: We are still only offering two submission groups, which means youth and young adults and high school students will now submit in the same submission group. Middle School students can only submit in the Walk in Our Shoes Category.

Directing Change is part of statewide efforts to prevent suicide, reduce stigma and discrimination related to mental illness, and to promote the mental health and wellness of students. These initiatives are funded by counties through the Mental Health Services Act (Prop 63) and administered by the California Mental Health Services Authority (CalMHSA), an organization of county governments working to improve mental health outcomes for individuals, families and communities.
